Contract Documents for Electrical work may be obtained at the office of J.M. Rotz Engineering Co. Inc., 2828 High School Road, Indianapolis, Indiana 46224. Each bidder may obtain one set of documents for a deposit of $30. Deposit checks Shall be made payable to The Housing Authority of the City of Indianapolis. Deposit will be refunded upon return of the documents in good condition to the Engineer within ten days after bid opening. Additional sets of Contract Documents are available at a cost of $15 per set, nonrefundable. No partial sets will be
issued.
Bids shall be executed in accordance with Indiana State Board of Accounts Form 96 with Non-Collusion Affidavit, be accompanied by Questionniare Form 96-A, and by a bond or certified check (“bid security”) made payable to the Authority in an amount equal to five percent (5%) of the total bid price, all of which documents shall be delivered in a sealed envelope .showing the bidder’s name and address and the bid item. To be eligible for an aivard of a contract under this invitation, all bidders or subcontractors must comply with Part I or Part II and Part HI of the Bid Conditions of Marion
County.
All bidders are required to submit with their bids the Bidders’s Certification Form and all subcontractors must submit the Subcontractor’s Certification Form with their bid to the Prime Contractor.
