Social security numbers for tax return forms Taxpayers claiming a dependent who is-at least five years old must include that dependent’s taxpayer identification number on their return. To get a social security number, a dependent must have evidence of: 1) date of birth, 2) US citizenship or legal alien status, and 3) identity. A public, hospital, or religious birth record is generally sufficient for first two items. A school ret a and certain other documents can be used for proof of identity. For more information, taxpayers may ask at any Social Security Administration office for the free leaflet, “Applying for Social Security Number. ” Panel to be featured at AAUW meeting The Warsaw Branch of the American Association of University Women (AAUW) will feature Sydney Ellis, Beverly Blood and Carolyn Mock, as a panel on the topic of the “Emotional and Social Needs of Children.” Ellis is an attendance officer, Blood, a school psychologist and Mock an assistant principal with Warsaw Schools.
