Ist, 1883 I wish to call Vic attention of the Medical prof ession of Jasper County to some fads relative to the filling of blanks of Births, [Deaths, and Diseases dangerous to public health. The law specifies that blanks should be relumed within fifteen days after the occurrence of a birth , or death, etc. If they are sent in at thclendof each month that will fill aU.fhe requirements of this office, and if done promptly will Save much trouble ? and some expense. It is earnestly urged upon all physicians, ‘mid-wives, and householders, to make returns of all cases coming under their care. Collect your information at the lime you attend the case, and be sure and gel all the facts so as to make a full report. Do not omit '“Nationality/’ “Place of Birth/’ nor “Age/’ of parents. Do Pol omit any thing. Some of the items may appear useless, and even superfluous, but other Stale Boards of Health have adopted them and find a complete record cannot be made without them. Our state in adopting them starts where the others left off. When persons are natives of the United Stales use the ward. “Native’’ in giving Nationality. A little carton the pari of each, will enable the Stale Department to compile vital Statistics, that, it. is hoped, may he\ a benefit to the people of the stale at large. 1 desire that all will cheerfully comply with the law, and aid in making the health Slatsi sties of our County complete, so that we may know whether Jasper county is the healthiest county in the stale or not. All reporldtforwarded by mail will be receipted for by postal card/so that all may hibw that their reports have been received. ‘ LB.'WASHBURN, M,D„ Health Officer, Jasper County- . wnan HIRAM SIBLEY ft CO.
