1. Get application blank at any inspection station, bank or Farm Bureau. 2. Attach the cover of your old “A” book to the application and fill out as directed under “Instruction.” 3. If you have lost your “A” book cover, use State Certificate of Registration (small white card about 2x4). 4. Mail to your local Ration Board as soon as completed. Ration Board No. 37-1
