The Indiana State Department of Health, Division of Long Term Care, is currently seeking to fill several positions, of Public Health Nurse Surveyor 3. The ideal candidate will be responsible tor surveying nursing homes and related facilities to ensure state licensure and federal certification requirements are met Conduct annual, follow-up surveys and compliant investigations. Surveyors are needed in the following counties Marion, Hendricks, Clinton, Johnson, Morgan, Hamilton, Hancock, Montgomery and Boone. Extensive travel is also a requirement with these positions. Minimum qualifications include; possession of a valid registered nurse license issued by the Indiana Health Professions Bureau, one (1) year full-time professional experience as a registered nurse in a health facility or hospital.
