O-D Registration For Kindergarten Set All children who will be five (5) years old on or before June 1, 1996, need to attend kindergarten screening at Oregon-Davis Schools, on Thursday, April 11. Parents should call the Elementary School at (219) 867-2711 to set up an appointment for screening, which will be from 8:15 a.m. to 7:15 p.m. Parents will need to bring with them a legal (not hospital) birth certificate and immunization record. The birth certificate may be obtained by contacting the Health Department in the county in which the child was born. If immunization is not now complete, it must be completed before Sept. 1, 1996.
