VIOLATION OF ROUTINE COLIFORM TESTING SCHEDULE As a public water supply we failed to meet the monitoring and reporting requirements of the state drinking water regulations for coliform testing during May. We are required to have water samples from our distributions system routinely analyzed for bacterial (coliform) contamination. These analyses are necessary to assure that the drinking water consistently meets the state health standards established by the Indiana Department of Environmental Management (IDEM). We did not have the required number of water samples analyzed during the period(s) stated. We regret this omission and will assure that the sampling and reporting of results are performed in the future. We did not receive notification from the state laboratory so that we could send another sample within the month of May, 1994. Town of Walkerton David Bolze, 7/25/94 219-586-3711 5271017 j2B
