All necessary labor and equipment to install, hook-up, erect, and connect thirty-five 175 watt Mercury Vapor Luminaires to be installed and placed on Monroe Street in the City of Decatur, Indiana between The West side of Third - Street and the West side of Thirteenth Street, and also all necessary labor and equipment to install, hook-up and connect fifty 175 Watt Mercury Vapor Luminaires to be installed and placed on Mercer Avenue in the City of Deactur, Indiana between Second Street (Five Points) and the City Limits. Said installations shall be at approximate 100 foot intervals at such places as wlj.l be marked by the City of Decatur, Indiana. 2 All necessary material and supplies for t he said Eightyfive (Fifty on Mercer and Thir-ty-five on Monroe) 175 Watt Mercury Vapor Luminaires, pursuant to the material requirements, specifications, and descriptions on file in the office of the Clerk-Treasurer of the City of Decatur, Indiana. 3. A combination bld. which would be the labor set out in number 1 above and the material and supplies set out in number 2 above. 4. Bidder shall state when he can start work and give approximate ■date of completion, and/or date of delivery of said material. Proposals or bids shall be properly and completely executed on State Board of Accounts Form No. 96, with Non-collusion affidavit as required by the Statutes of the State of Indiana, and must be accompanied by questionnaire Form No. 96A, State Board of Accounts for all bids of Five Thousand Dollars ($5,000.00) or more, as required by law. Every bld or proposal shall he accompanied Uy an acceptable bidder’s bond or certified or cashier’s check payable to the City of Decatur. Indiana, in an amount of not less than 5% of the bid submitted as a guarantee that if the proposal is accepted a contract will be entered into and the performance of the contract properly secured. A 9-year-old Kokomo boy was injured fatally Thursday night. Michael K. Duncan was killed when a car driven by Richard Gillett, 37, Kokomo, went off a curve Thursday night on U.S. 35 south of Galveston. The car rolled over three times, throwing out all of the occupants, and landed on its top. Gillett was hurt seriously and . his three children were injured. ( They are Richard, Jr., 13; Terry, J », and Maifc, 6. The Gilletts and the Duncans are neighbors.
