Social Security Quiz Answers Many Queries
(Editor’s note: Do you have any questions about Social Security? We all do. don’t we? Send any question you might have to Social Security Quiz, c/o Daily Democrat. You’ll find your answer in this column, which is published each Saturday.) ' 1, Q—When I come in to file for my retirement benefits must I bring a statement of my wages? If so, how far back? A—Yes. it will speed up the processing of your claim if you can bring it your W-2, Withholding Tax Statement, for the past calendar year. You should also bring in a list of the names and addresses of your employers and the approximate amount of your wages during the current year* 2. Q—My employer is retiring
me on June 30,1958, at age 69. Assuming I don’t work any more and that I made over 15,000 annually the last 10 years, what win my benefit be at age 66? Please explain your answer. A—slll. You were credited with maximum earnings of $3600 a year 1951 through 1954 and $4200 a year 1955 through 1958. Since 5 low years are dropped in figuring average monthly earnings, the years 1959 through 1963 would be dropped. This results in an average monthly wage of $325 a month. The monthly benefit amount is based on this average monthly wage and would therefore be slll. 3. Q—What proof of age is needed when I file my claim for benefits? A—ls you have a birth certificate, bring that along. If you do not have this certificate, here are a few other documents that may be acceptable—your baptismal certificate; the family Bible of names of members have been recorded for some years; an insurance policy that was taken out many years ago; if you were in the service, your service discharge; your oldest registration certificate available. 4. Q—My wife and I have been receiving a combined social security check based upon my work. Last month she passed away. Will it be all right if I cash the check, even though both our names appear on it? A—No, you should not cash this check, but it should be returned either to the Treasury Department or to your nearest Social Security Office. The address of the Treasury Department is on the upper left hand comer of the envelope of the check. You will then be issued a separate check for the amount to which you are entitled yourself. 5. Q—What is acceptable proof of marriage. A—Any of the following are acceptable; the original certificate of marriage; a certified copy of, or statement as to church record of marriage; a certified copy of the public record of your marriage, a statement from your wife or husband on an approved social security form certifying your date and place of marriage. 6. Q—My wife and I have been . receiving a combined Social Secur- • ity check based upon my work. She died several months ago, and * Fhotifidd your Offiße'Wher"share 1 of the check was stopped. Now I , am wondering if I am entitled tc any death benefit? I A—No, you are not. A lump sum death, payment is payable only where the deceased individual had worked long enough to be insured under Social Security at the time of death. At the time of your death, for example, the lump sum of three times your benefit rate, 1 but not to exceed $255, will be paid to whomever pays the burial expenses. 7. Q—Where will I get the public record of my marriage? A—lt may be obtained from the clerk of the court in the city I or county where the marriage li- . cence was obtained, or the Bureau . of Vital Statistics of the State in ; which you were married, fees are generally charged for such rec- ■ ords.
