Geriatrics Is Growth Field When we moved to a new community a few years ago, it became necessary to get a new family physician. In looking through the yellow pages, I noted one who listed himself as a specialist in the medical care of the aging. Although I had been satisfied with a family practice specialist we had long used at our previous location, I had wondered what I would do if forced, by some circumstance, to change doctors. I must admit some age prejudice as a cause for this vague concern. I couldn’t see how a much younger doctor could empathize with the pains I felt nor the symptoms I worried about. It appears that the medical profession recognizes this concern on the part of older patients. The Journal of the American Medical Association reports (Sept. 15, ’B9) on an “aging game” being employed in the clinical medicine course at Duke University in which “self image chips” are used to help medical students envision themselves as aging and deciding what is most important to their well-being, ear plugs are used to simulate hearing loss, and safety goggles are coated with petroleum to simulate visual loss. The American Board, certifying organization for family practice, has led an effort to award a CAQ (certificate of added qualifications) to diplomates who meet postresidency training requirements and pass a written examination in geriatric medicine. patients is high. When he established private practice in Las Cruces, N.M. in 1969, geriatrics was just beginning to take hold as a specialty, and he decided to concentrate in that area in 1971. Dr. Cantu likes senior patients and his own aging enables him to understand them first hand. They come not so much for check-ups as with complaints, he says. They need to know the source of such complaints. Some will never be cured, he says, but many can be controlled with the proper balance of medications, contributing to a general feeling of well-being. Balance, however, is the key word. On each appointment, patients must undergo what Cantu calls “the bag test” (they must bring in all medications they are using). Because older people are nearer to the end, they wonder more about terminal conditions. Sometimes, I find, they are so worried that it is hard to accept a non-terminal diagnosis. The fretting is not as much over terminality as it is with the discomfort and inconvenience of the treatment they may be required to undergo and the distress they will cause their loved ones. This all means that a geriatrist must possess a good capacity for mental counsel. Among the ailments that “prey” on advancing age, the doctor says, are hypertension, arthritis, diabetes, prostate gland difficulty, emphysema, intestinal maladies, and sight and hearing impairment. I would not encourage a senior who has long-earned confidence in her/his family physician to change. But, if a change must be made anyway, I would recommend consideration of a geriatrist.

Crime prevention programs are also cost effective since it is basically just an matter of educating the citizens. “I would challenge you as a community to include in your comprehensive plan, a crime prevention unit,” she said. The Indiana Watch coordinator also shared the anonymous tip line system with the P.I.E. group. THE TIP LINE HAS proven that even the smallest of communities have crime and drug problems, Bowland said, according to the location from where the calls come in. As a former school teacher from a small, rural community, Bowland said she has found that no place is safe from drug problems, no matter how tiny the community. She said she has not plotted the location of all the anonymous tips, but she has seen that interstates are a pipeline for drug traffic into nearby cities and towns. Bowland said that once she receives information through the tip line, she routes the information back to local law enforcement

agencies or drug task forces. Usually, the information is used to build a case that is already being investigated by police, she said. One anonymous tip is not enough to have a person arrested, she said. The tip line number is 1-800-25-FED-UP. MEANWHILE, it was the annual meeting for the P.I.E. Coalition and committee reports were given on the 1990 activities for the one-year-old organization. Goals for 1991 include applying for more grants to finance P.I.E. projects, make P.LE. more noticeable throughout the county, and work on community assistance and support group efforts. New board members and officers were also selected. The 1991 officers are president Darrel Thomas, vice president Pat Braden, secretary Sandy Pruitt, and treasurer John Mishler. Board members are John Franklin, Jerry Hecko, Mike Cash, Eric Bemsee, Susan Murray, Justin Krider, Del Brewer, Joe Eubanks, Henry Sabetti, Susan Crosby and Louise Reinecke.
