Surgeon Answers Questions On Hospitol

Pago 3

To clear up questions on the new Putnam County Hospital to be built by Humana, Inc., the old Extendicare, Inc., the Banner-Graphic asked Dr. Cleon M. Schauwecker, local surgeon, to answer some of the most frequent inquiries about the new structure. Dr. Schauwecker consented to answer the questions with the stipulation that the answer be considered his and his alone, and not necessarily representing those of the medical staff of the hospital or the Hospital Board or the Putnam County Commissioners. QUESTIONS Q: Just what is a Proprietary Hospital? A: These arc hospitals built by companies whose prime business is the construction and management of hospitals. Like any company, these are operated for a profit, as that is the only way the company can stay in business. Most of the larger such companies have stock which is sold on the New York Stock Exchange. There are 5 chief companies who do this sort of thing, of which Humana is one. Q: You mentioned Humana. Who or what is that? A: That is the new' name for Extendicare. Extendicare changed its name about 2 weeks ago. It was done primarily because of the confusion with nursing homes. Extendicare was once in the nursing home business but no longer. The company is now purely in the Hospital construction and management business. Q: You mentioned there are 5 leading Proprietary Hospital companies. Why was Extendicare (Humana) picked? A: Acutally, they contacted us first—and this was simply because we were high on the list of hospitals in Indiana that needed extensive remodeling or a new hospital. This list is compiled by the Indiana State Board of Health, who sets the standards that an Indiana Hospital must meet. Our present hospital is deficient in several instances. Q: Were other hospital companies contacted? A: Not seriously. We looked so extensively at the Humana Corporation and were so pleased with what we found that we really saw no need to look further. Q: Just what do you mean when you say you checked Humana extensively? A: For one thing, well over 50 doctors were contacted personally who practice in a hospital built and managed by Humana. Also, the executive secretary of the American Medical Association was

contacted and the Hospital Division of the A. M. A. checked out the corporation— whether there had been complaints, the quality of their services, their relations with the community, etc. We received a most complimentary letter in response. A number of local people including members of the County Commissioners, County Council, Doctors, Nurses, our hospital administrator, and others visited 4 hospitals owned and managed by Humana. We were all very pleased and impressed with what we saw. I have letters from over 30 Mayors of towns where they have Hospitals located, and these were most complimentary. Of course the deciding factor was the quality of the management. They have truly impressed all of us most favorably. Now, as you can see, it would just be impossible to investigate many companies this extensively—time just would not permit. However, 1 do want it made clear, that there could well be other companies just as good, or even better; but, we thought it better to investigate one company quite thoroughly rather than do a half-w'ay job on several. The conclusions reached were unanimous-no single individual tried to persuade anyone else. I suppose I am saying this because I fully expect that there will be some criticism-about something or other about the Hospitalso, all I can say is that we tried our best. Q: What are the main differences between a proprietary hospital and the conventional hospitals in Indiana? A: As to the appearance, and the method of patient care, etc-there are no differences. A patient would never know if he were in a Church -affiliated hospital, a County hospital, or a Proprietary Hospital. The big differences are two. First, the Proprietary hospital is built and managed by a corporation. This means that the company puts up the money rather than a Church, County or City, etc. In our case. Putnam County will not spend one cent for tfiis new hospital-Humana finances it all. Secqndly, while our hospital will have an administrator as it does now, all really major decisions are made by the company. For example, if we wish to purchase a new x-ray machine now, there have to be bids, etc. Humana simply places the order-period. Also, because of its size and number of hospitals, they purchase all the supplies and equipment in such huge quantities that this produces large savings which are passed on to the patient.

Q: Do you mean that Putnam County taxpayers will not pay any portion of this new hospital? A: This is true. As I understand it, the Accumulative building fund tax will be removed, and the funds already accumulated will be removed, and the funds already accumulated will be returned to the county. Incidentally, being a “forprofit” hospital, they will be taxed-just like I.B.M. or Mallory, etc. I have seen their proposed budget for their first year of operation, and they intend to set aside 5100,000 for local and state taxes. Q: Won’t the hospital rates be much higher since they must pay taxes and expect to make a profit, etc. A: Again, I have seen their project or anticipated rate structure. A room with 2 beds in our hospital now costs $49.00 per day. Humana expects their rates to be between 552.00 and 555.00. Now, you must realize that this difference is not profit for the company. The new hospital will have so many things the present building does not have-and these all cost money to provide. For example, if this hospital proves to be like those we examined-and 1 fully expect that it will, the patient will have the following he does not have now. The entire building will be carpeted-a great noise reducer, and certainly appreciated by the sick patient. All rooms have color television mounted on the wall-with controls at the bedside. All rooms have a telephone for each patient, and with this same phone, one can dial the nurses’ station and carry on a conversation. This is a great time-saver for the nurses and a great convience for the patient. Each room will have its own toilet and shower facilities. The emergency room area will be about 3 to 4 times larger; the laboratory area will be about doubled in space-hence, many more tests can be done locally rather than sending them to

Terre Haute-and this will save much time. Actually, every department will be geared to run more efficiently and hence provide better patient care. Q: Where will the hospital be built? A: At this point, nobody knows. Humana Corporation has one group of men who do nothing but decide the places to build the new hospitals and then proceed with the development of the areas. This decision will be made in the next one to two weeks. W'hile the company will actually make the decision, they will consult with local individuals. All we really know is that our contract with them calls for the Hospital to be built in Greencastle, or in an “annexable area”. This is primarily so that the sewage can be attached to the present system. Q: What happens to the local employees of the hospital? A: The same rules will apply to the local doctors as are now in effect. Incidently, we doctors do hope for one change—that more doctors will be attracted to the area. Incidentally, at our last social meeting, the doctors and their wives of Putnam County invited 3 doctors and their wives plus a medical student and his fiance to our social meeting. Needless to say. we all tried very hard to convince them to consider this County and we have been informed that 2 doctors are definitely considering coming to Greencastle. They will complete their training in June. We only hope they do not change their minds. It goes without saying, that the prospects of a new hospital certainly was an important factor in their decisions. Now, I am sure there are many unanswered questions. If you have a question, write or call the Banner-Graphic and perhaps there can be another question and answer session. If I do not have the answer, I will certainly try my best to get it for you.
